Hill's Pet Nutrition added ActivBiome+ Kidney Defense, a proprietary prebiotic blend, to its entire k/d prescription diet line for cats and dogs.
The blend targets gut-derived uremic toxins, protein-bound compounds that promote oxidative injury, inflammation, and fibrosis in renal tissue and are elevated in dogs and cats with chronic kidney disease at all stages. Traditional dietary therapy for CKD controls protein and phosphorus intake but has limited ability to remove these toxins, which are difficult to clear via conventional approaches. The company spent more than a decade researching the gut microbiome's role in CKD progression and identified an opportunity to redirect microbial activity from proteolytic to saccharolytic fermentation, reducing uremic toxin production at the source.
The gut-kidney axis Hill's is betting on
CKD is associated with a specific type of microbial dysfunction: predominantly proteolytic bacteria that ferment amino acids and produce high levels of harmful uremic toxins. Hill's researchers, working with partners at the Harvard T.H. Chan School of Public Health's One Health Microbiome Resource, a database of over 2,000 canine and feline gut microbiome samples, developed a proprietary fiber blend with three key components the company does not name publicly.
A dose-response study in dogs showed that a moderate concentration of the fiber blend yielded the greatest reduction in uremic toxins, though all tested concentrations significantly improved uremic toxin levels and antioxidant markers in dogs with early CKD compared to control food. In crossover studies, cats with early renal disease fed k/d with the ActivBiome+ blend had lower uremic toxin levels compared to controls and showed significant weight gain, addressing one of the hardest clinical problems in feline CKD management.

What this changes for the associate two years out
The addition creates a compliance-light option for early-stage cases where full diet-change recommendations routinely fail. You still have the controlled-phosphorus, highly-digestible-protein foundation of the k/d line, plus Enhanced Appetite Trigger technology Hill's says stimulates appetite and encourages food intake. But the prebiotic blend now addresses the uremic toxin cycle without requiring owners to accept a prescription diet in the window where most abandon it.
The blend is included across all k/d products: standard k/d, k/d + j/d for kidney and mobility, and the k/d + Derm Complete and k/d + z/d Hydrolyzed formulas designed for patients with both kidney disease and food allergies. The inventory question for owner-DVMs
This is still a prescription diet, not a standalone supplement, so it doesn't create a new SKU category to stock or a new client-communication problem about when to use a supplement versus a food. But it does change the value proposition of the k/d line: you're now selling a gut-microbiome intervention on top of the traditional renal-diet benefits, which means your team needs to be able to explain what "reduces uremic toxin production at the source" actually means to an owner deciding whether to make the switch.
The company reports that k/d clinical nutrition is shown to add 1.5 years on average in dogs and 2 years on average in cats after CKD diagnosis without concurrent conditions. The ActivBiome+ addition is Hill's bet that the microbiome piece, reducing toxins that traditional dietary therapy can't clear, closes part of the gap between what early intervention should deliver and what compliance rates actually allow.
Our read
The gut-kidney axis research is real, and the uremic toxin problem is one traditional renal diets don't solve well. The crossover-study weight gain in cats is the number that matters most, if you can get early-stage CKD cats to not only tolerate but gain weight on a therapeutic food, you've addressed the failure mode that kills most diet-change attempts in the first month.
What we're watching: whether the prebiotic story becomes a tool associates actually use in the exam room when an owner pushes back on the diet change, or whether it stays a label claim that doesn't move the compliance needle. The product works only if the cat eats it, and Hill's already built E.A.T. technology to address that piece. The ActivBiome+ addition is the company's answer to the question of what happens after you get the cat to eat, whether you can intervene on the toxin cycle early enough that the diet change delivers the survival benefit the studies promise.
The blend is in the food now. The test is whether it changes the conversation in the exam room when you're trying to get an owner to make the switch before the cat is in late-stage uremia and the window has closed.
